Personally, my favorite type of level is just one huge level with random enemy spawns. The game i'm working on right now (ok it's just a concept) is one where you are the immune system for a human, so you can go anywhere in the entire body. I love it when you have no limits except the end of the world.
A big problem with something like that is that if the world too big, you need quicker forms of transportation, such as cars and planes or even teleportation. Unless there is a quick way of traveling, try to keep people from having to travel to opposite ends of the map. That makes games really annoying.
