I was playing Skyrim once and accidently killed my companion in the snow.
I was so sad I stopped playing for months.

Stupid things I've done throughout video games? I've got quite a few.
Chrono Cross-I decided to play through half the game without saving... then I died.
SSBB-I was testing out a trick on a level 9 CPU so I jumped off the stage to kill him only to commit suicide.
Those 2 things happened 1-2 years ago... So I can't call them my most recent deaths.
